To prepare for your appointment please fill out this form. We will share all of your details with your stylist before your appointment. If you are getting an error message due to exceeding the photo size and/or number limit, please email your inspiration photos to lasvegas@revealhm.com instead.

If your bridal party should have a uniform look, please attach photos of your vision. If each bridesmaid / bridal party member has an individual look, please organize it by name in a pdf. Your party may also show inspiration photos to the artists on the day of the appointment. The max file upload size is 50 mb.
Let us know if you have allergies to any item that could be found in a hair and/or makeup product.
As a friendly reminder, any parking costs should be reimbursed directly to your artist via cash, Venmo, or Zelle.
